Transition plans in Individualized Education Programs (IEPs) are crucial for ensuring that students with disabilities successfully move from school to post-school activities. These plans help identify the student’s strengths, preferences, and interests, guiding them toward appropriate career, vocational, and educational goals. By setting clear objectives and outlining the steps needed to achieve them, transition plans provide a roadmap for students as they prepare for adulthood.

The Individuals with Disabilities Education Act (IDEA) mandates that transition plans be created by age 16; however, most states have adopted the age of 14 for the initiation of these plans.
